The Wesley Community is seeking an Registered Nurse (RN) MDS Coordinator/Manager! This is a Full Time Day, Monday - Friday, opportunity.

WHAT WILL I DO?

As an RN MDS Coordinator/Manager, supporting the mission of Wesley Community will include the following responsibilities:

  • Acting as the main reference person for the interpretation of MDS regulations according to the RAI User Manual to the Interdisciplinary Team and NYS Department of Health.
  • Responding to Medicare appeals as occur.
  • Assuring the completion of the MDS process from scheduling OBRA as well as OMRA assessments (both scheduled and unscheduled) through transmission.
  • Monitoring resident condition, participation in therapies and schedule MDS assessments to facilitate casemix. 
  • Facilitating weekly Medicare/HMO meetings with nursing/social services/therapy to foster communication re: resident participation/progress in therapy/potential discharge/etc.
  • Educating new nursing staff and necessary departments on how to complete an MDS.

WHAT IS REQUIRED OF ME FOR THIS ROLE?

  • Diploma from an accredited School of Nursing.
  • Bachelor of Science in Nursing preferred.
  • RAC-CT Certification Preferred
  • Extensive knowledge of MDS 3.0.
  • Current RN registration with the New York State Board of Regents.
  • Head Nurse, Supervisory and/or Administrative experience.
  • Geriatric nursing experience.
